New Patient Registration
If you live within our practice boundary you are welcome to register with us and our reception staff will be happy to guide you through the procedure, although there may be a surgery closer to you which you may want to consider. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill, along with one form of photo ID such as a passport or driving license.
We accept new registrations during the following times each day: 10:00 - 12:00, and 13:00 - 16:00. If you arrive outside of these times you may be asked to return.
Following changes to the NHS Registration form we are no longer able to accept registrations via the website.
If you wish, please call into the surgery in person, and we will be happy to help you.
Medical treatment is available from the date of registration (which is when we register you, not when you hand the forms in. Registration can take up to one week) so please ensure you have sufficient medication before leaving your previous surgery.
You can drop into the surgery to collect the registration forms, or download the forms below to complete and print out. Every adult must present to the surgery with their forms, photo ID and proof of address. Your NHS number can be found on your NHS App, letters from an NHS service or your current GP surgery can provide this.
